Conditions of Accessing

Referral Process

Adults who have a severe and persistent mental illness are eligible to be referred to the Independent Living Program. Eligible people are referred by clinicians, social workers and nursing staff to the Bentley Health Service. Referrals are also accepted outside the Bentley Health Service. Staff from the participating agencies are particularly aware of the needs of those people, marginalised from mainstream services, eg., Aboriginal people, people from a non-English speaking background, and people experiencing homelessness.

To enter the program a person must agree to adhere to the conditions laid down by their case manager.

The person must also agree to a Housing Officer from St Bartholomew’s House keeping in contact with them, and to abide by the conditions outlined in the lease agreement.

No person can enter the program unless they have a designated Case Manager.

Services Provided

Community Independent Living
The provision of accommodation in houses and units owned by the Ministry of Housing and leased by St Bartholomew’s House for people with severe persistent mental illnesses. Support is provided, at least fortnightly, to ensure residents have an introduction and continuing interaction with their local community.
