KEY ACTIVITIES - ASSISTANCE WITH CARE AND HOUSING FOR THE AGED PROGRAM (ACHA)

The Program

The St Bartholomew's House ACHA program is committed to providing sustainable, affordable, accessible and high quality aged care.

The ACHA Case Manager helps clients find affordable, secure housing and obtain immediate health and care services. Long-term support is coordinated using community services for household help, nursing support, meals on wheels, transport and personal care.

Advocacy

An advocate is a person who, with the client's authority, represents his/her interests. An advocate can support and assist clients in dealing with the public housing authority, or other government and non-government organizations.

The ACHA Program targets

Anyone approximately 50 years and over who is:

Referrals

Referrals to other organizations may be made in order to assist clients to remain living independently within the community, thereby avoiding premature entry into residential care due to housing related issues.

Referrals can be made to the Aged Care Assessment Team in order to provided in-home community support and medical appointments to avoid complex health issues impacting on the client's quality of life.
